Will Jennings, médecin anesthésiste, vit dans le Mississippi avec sa femme, Karen, et Abby, leur petite fille de cinq ans, qui est diabétique. Parti quelques jours pour un congrès médical, Will se trouve pris au piège d'une machination machiavélique.
Aidé de sa femme Cheryl, et de son cousin Huey, un simple d'esprit, Joe Hickey, un criminel aussi intelligent que déterminé, a mis au point le « kidnapping parfait » : il profite du départ du père pour enlever l'enfant et séquestrer la mère.
